Here you can find forms you may need to use at various points in your studies.
Please be sure to read the information at the top of each form.
- Any year of study: Request to enroll, un-enroll, or change your enrollment in a course
- Any year of study: Request to transfer to a different study track
- First and second years only: Request to enroll in or un-enroll from a specialization
- Third year only: Request to transfer to a different specialization and select a general departmental course
- Fourth year only: Degree eligibility report
